(Including Visiting Scholars, English Language Institute, and Exchange Students)
This student health insurance plan is underwritten by Blue Cross Blue Shield of Florida and is mandatory for all students in the categories listed below. Please check with your appropriate department to make application and payment to the school. Upon receipt of the enrollment and eligibility lists from the school, you will receive an ID card in a timely manner. Student health insurance is mandatory for all students in the categories listed below. The student health insurance plan, underwritten by Blue Cross and Blue Shield of Florida, meets all coverage requirements. Please check with your appropriate department to make application and payment to the school. Upon receipt of the enrollment and receipt of the eligibility lists from the school, you will receive an ID card in a timely manner. International Students, Visiting Scholars,
Exchange Students and English Language Institute Students:
International (Regular) Students:
Students who pay the semi-annual payment for Fall must pay the second semi-annual premium coverage February 20, 2010 through August 19, 2010 before they can register for Spring classes. They can enroll using the Spring/Summer Enrollment Form
Students who take classes for the first time beginning in Spring, must have insurance coverage for January 1, 2010 through August 19, 2010 before they can register for Spring classes – using the NEW Spring Enrollment Form. Students who take classes for the first time beginning in Summer must have insurance coverage for May 1, 2010 through August 19, 2010 before they can register for Summer classes. Separate enrollment forms for New Spring and New Summer Students will be posted at a later date.

Enroll Online*
When enrolling online via credit card, the on-line application will ask for your social security number. DO NOT use your social security number, but put your Panther ID number in that space, including additional zero’s (0’s) to make it a nine (9) digit number. Please put the zeros on the end of the number. Once your credit card information has been processed, your application will be approved and sent to FIU to clear you for registration the day you enroll.
*Please note, there is an additional administrative fee for enrolling on-line. The annual cost is $1,832 if paid by cashier’s check or money order, vs. the cost on-line via credit card is $1,887 and for the first semi-annual payment, the cost is $916 if paid by cashiers check or money order, or $944 if you pay on-line. You may pay for your insurance by bringing cashier’s check or money order to the Student Health Center on campus, along with your application. Remember, there is no additional charge if you pay by cashiers check or money order. You may also enroll by wire transfer for an additional $15.00 fee. Wiring instructions are as follows:

Visiting Scholars
Scholars must pay the full premium for the length of their authorized stay. Total premium must be paid at the time of enrollment. Dependents may also be enrolled. Follow the instructions on the form, which you can link to below.

Exchange Students
Enrollment in the insurance should be coordinated with the Office of Education Abroad, Florida International University, University Park, DM 441, 11200 SW 8th Street, Miami, FL 33199. If you have specific questions not addressed here, you may call the Office of Education Abroad at 305-348-1913. Rates for the insurance coverage coincide with your enrollment periods, as described in the provided enrollment form.
Payment must be made to the Office of Education Abroad.

English Language Institute
English Language Institute Students and their dependents will be allowed to purchase the insurance on a pro-rated basis for the period of time they will be enrolled at the University. The English Language Insitute offers Four Ten Week Terms Per year broken down by 5 weeks. Full Term and Half Sessions begin in January, April, July and October. Second Half Sessions begin February, May, August and November. If you sign up for one period and decide to enroll for a second session or beyond, you are responsible for timely enrollment and renewal of your insurance premium. At the time of enrollment, please submit a cashier's check or money order, payable to Blue Cross and Blue Shield of Florida, to The English Language Institute, 11200 SW 8th St., Labor Center 101, Miami, FL 33199, along with the completed form below.

Graduate Assistants (Dependent Enrollment Only)
Graduate Assistants must work with the Administrative Services at Florida International University Graduate School, UP Campus, PC 230 to sign up for the insurance. They will assist you with what is needed to enroll in the insurance.
If you are enrolled in the 09-10 Student Health Plan through Blue Cross and Blue Shield of Florida, you are also free to enroll your dependents, so long as they are covered for the same dates. Dependents must have the same coverage and enrollment dates as the Graduate Assistants. To enroll your dependents in this health insurance program, fill out a dependent enrollment form, which you can print and follow the payment instructions on the form.

Medical Students
including information on Disability Insurance and Needlestick coverage
All medical students are required to be continually covered by a health insurance plan throughout their enrollment. Needlestick coverage is included as part of your plan design. Students who do not provide annual documentation of comparable alternative coverage will be automatically enrolled in the plan. Coverage for dependents is also available on a voluntary basis. Dependents must have the same coverage and enrollment dates as the medical students. Follow the payment instructions on the form.

Disability Insurance for Medical Students
Medical students may enroll in the Disability AMA Med Plus Advantage Plan. Students are required to enroll through the College of Medicine Administrative Offices. Additional information is provided below. This coverage includes:
- Disability Income Protection
- Long Term Disability Benefit of $1,500 Per Month
- $200,000 Student Loan Repayment
- Benefits Until Retirement While Disabled (SSNRA)
- 90 Day Waiting Period
- Lump sum benefit of $5,000
